Membership Terms & Conditions

1. Membership Duration - All memberships have a initial 6 month commitment with a $25 start up fee.
2. Approved reasons for cancellation within the 1st 6 months - death, disability, illness or injury with a doctors note, change in residence greater than 30 miles from our location, being called away for activity duty in the US Military. After your 6 month commitment you may cancel your membership at any time for any reason. Cancellations are only accepted via our cancellation form (link is located on our website) If a member engages in conduct that Monumental Med Spa deems objectionable or contrary to the spa's values and policies, Monumental Med Spa reserves the right to terminate the individual's membership.
3. Transformation Plan - Includes added 1 Complimentary Filler Syringe per Membership period. Subscription price is $2 first 6-month subscription period. $1 every subsequent 6-month membership renewal.
4. Membership Payments - Membership fees are due in advance and cover benefits for the following month. Failure to make a payment will result in a suspension of membership benefits and discounted pricing until the account is brought current.
5. Lapse in Membership Payments - Should there be a lapse in monthly membership payments, access to membership benefits, including discounted pricing, will be suspended until payments are resumed.
6. Service Recommendations - While a variety of services are available as part of our monthly memberships, not all services are recommended for monthly use. Members should adhere to recommended usage frequencies to ensure the best results and maintain overall skin health.
7. Refund Policy for Membership Payments - Monumental Med Spa enforces a non-refundable policy for membership payments. Once a monthly payment is made, it is acknowledged as final. In the case of a member requesting a refund post-payment, please note that refunds will not be issued under any circumstances. This policy is in place to maintain the integrity and financial stability of our membership program.
8. Consent to Store Billing and Payment Information - By becoming a member of Monumental Med Spa, you agree to allow us to securely store your billing and payment information. This information will be used exclusively for processing membership fees and other services you avail of at our spa. We adhere to stringent privacy and security standards to ensure your financial data is protected and confidential. Your continued membership signifies your consent to this policy.
9. Obligation to Maintain Active Payment Method - As a member of Monumental Med Spa, it is your responsibility to ensure that an active and valid credit or debit card is maintained on file at all times. This is essential for the uninterrupted processing of your monthly membership fees and any additional services you may use. Failure to keep an active card on file could result in a suspension of membership benefits until the issue is resolved. Your attention to this matter helps us provide continuous and seamless service to you as a valued member
10. Restriction on Using Gift Cards, Coupons, and Points for Membership Fees - Members of Monumental Med Spa are advised that gift cards, coupons, and points are not permissible for use towards monthly membership fees. These benefits are exclusively available for use on our range of services and products outside of the membership fee structure. It remains the member's responsibility to maintain an active and valid credit or debit card on file for the uninterrupted processing of monthly membership dues. This policy ensures a consistent and equitable approach to membership fee management for all members.
11. Memberships are non-transferrable. Membership Privileges and Usage Restriction: The services and benefits provided under this membership are exclusively for the personal use of the registered member only. These privileges are non-transferable and cannot be shared, loaned, or extended to friends, family members, or any other individuals. The member must be present and directly engaged in the use of services or benefits at all times. Any unauthorized sharing or transferring of membership privileges will be considered a violation of the terms and conditions of this membership, and may result in termination of the membership without refund and/or other penalties as deemed appropriate by the service provider.
12. Monthly Service Redemption and No Rollover Policy: Members must redeem their entitled services within the corresponding month of their membership period. Any unused services or benefits will not be carried over or accumulated into subsequent months. It is the responsibility of the member to ensure timely redemption of their monthly services. Failure to utilize these services within the designated month will result in the forfeiture of the unused portion without the possibility of refund, credit, or transfer to future months. This policy is in place to encourage regular and timely use of the membership benefits and to maintain the quality and availability of services for all members.
